Home Care Provider Referral Portal

The Home Care Provider Referral Portal displays information about individuals approved for home care services on an electronic referral list web page. Home care agencies use the referral page to view general beneficiary information for individuals in need of homemaking and personal assistance services to identify individuals they can serve.

The Home Care Provider Referral Portal is for beneficiaries in certain Medicaid fee for service programs:

Core community services through the Department of Human Services (DHS)
Office for Healthy Aging (OHA) core community services
Preventive services
Habilitation community services

The Department of Behavioral Health, Developmental Disabilities and Hospitals (BHDDH); OHA@Home Cost Share program; pediatrics; and Medicaid managed care organizations do not use the referral portal at this time.

During the first week of each month, OHHS will post updated data summarizing activity from the referral portal.
