Please note: Due to file restrictions on this website for posting non-PDF documents, we kindly ask that you contact Debbie Morales (Deborah.Morales@ohhs.ri.gov) for attachments B and D of the Comprehensive AE Application. Attachments are no longer embedded in the AE Application file posted on this website. We kindly request that Applicants not contact individual EOHHS employees, contractor or subcontractor parties to EOHHS for inquiries about the application during the application submission and review process. Applicants should follow procedures and protocols stated within this application for correct submission.

Correction Note: (12/1/17):  There was an error in Attachment D’s ‘Self-Assessment’ section with the scoring scale. A score of one (1) is low, a score of five (5) is high on the corrected scale. Please email Debbie Morales (Deborah.Morales@ohhs.ri.gov) if you do not have the corrected version of Attachment D. 



Correction Note: (12/19/17):  There was an error in Attachment D’s tab ‘Domain 2: Governance’ section with a couple of the pull down tabs. These have been fixed. Please email Debbie Morales (Deborah.Morales@ohhs.ri.gov) if you do not have the corrected version of Attachment D.

Correction Note: (1/10/18):  There was an update in Attachment D’s tab ‘Domain 1: Provider Base' section. This update addresses the concern of having a single option for provider type. The updated attachment D provides entities with multiple provider types an ability to capture such information succinctly and should be utilized if needed.  The previous version of Attachment D is acceptable if a single provider type is appropriate for the entity. Please email Debbie Morales (Deborah.Morales@ohhs.ri.gov) if you do not have the corrected version of Attachment D.

Correction Note: (1/18/18):  There was an update in Attachment D. This update addresses a formatting error, and allows Governing Committee member names to be entered for single entity applicants (Domain 2.2). The previous version of Attachment D (v1.10.18) is acceptable for a multiple entity applicant. Please email Maria Narishkin (Maria.Narishkin@ohhs.ri.gov) or Debbie Morales (Deborah.Morales@ohhs.ri.gov) if you do not have the corrected version of Attachment D.

Correction Note: (2/6/18):  There was an update in Attachment D. This update addresses a drop-down menu error on tab 3, part B, provider type. Please enter the RI licensure type for the clinicians listed. The previous version of Attachment D (v1.17.18) may still be used. Please email Maria Narishkin (Maria.Narishkin@ohhs.ri.gov) or Debbie Morales (Deborah.Morales@ohhs.ri.gov) if you wish tio have the corrected version of Attachment D.

Application Information Session 

Application information session will be December 4th from 2-4 p.m. at DXC's 2nd floor conference room (Room 201, 301 Metro Center Blvd, Warwick, RI 02886). This meeting will be to review the application and associated attachments for correct submission. While participants will not be allowed to ask specific questions related to their submission, it should be noted that during the Public Comment section of this public meeting participants may state a comment about the application and the AE team may respond in a posted FAQ on the EOHHS website. The team will not respond to comments at the meeting.

Application Submission Information

Comprehensive AE applications are due no later than 3 p.m. on February 15, 2018 to January Angeles, Deputy Medicaid Program Director, Managed Care and Oversight.  Applications shall be hand-delivered no later than 3 p.m. on  February 15th to: January Angeles Deputy Medicaid Program Director, Managed Care and Oversight Rhode Island Executive Office of Health and Human Services, Virks Building 3 West Road Cranston, Rhode Island 02920. Electronic submission materials should be emailed to January Angeles (January.Angeles@ohhs.ri.gov).

The applicant will submit the following documents to EOHHS in the following order: 

  1. Letter of Transmittal
  2. Attachment B- AE Application Attachment Summary Checklist
  3. Proposal Summary, References, Technical Proposal and associated attachments
  4. Attachment C-Assurances and Attestations
  5. Attachment D-Excel Application Template Tool.

Late or incomplete applications will not be considered by the Review Committee.
