A Rhode Island Health Home utilizes an interdisciplinary team approach to address recipients' physical and behavioral health needs through the provision of comprehensive care management; care coordination and health promotion; comprehensive transitional care, including appropriate follow-up from inpatient to other settings; patient and family support; and referral to community and social services if relevant. Rhode Island Health Home reimburses at $442.21 for the monthly case rate using procedure code: H0046 U8.
