  • 6/15/2015 - Below is Rhode Island's Transition Plan for Home and Community Based Services that was submitted to the Center for Medicare and Medicaid Services (CMS) on June 24, 2015.  The Transition Plan is subject to CMS approval.

    Enclosed in this version is documentation of the Public Hearings that occurred on April 30, 2015 and May 5, 2015 and the State's responses to the 30 day public comment process ending on May 29, 2015.  HCBS Transition Plan  
  • 4/14/2015 - Below is Rhode Island's Transition Plan for Home and Community Based Services.  This will be labeled a Draft until all public comments are made and then incorporated into the plan.  Last day of accepting comments will be close of business May 30, 2015.

    Also below is the Public Notice forum for Public Comment Thursday, April 30, 2015 at 9:00 am at the Hewlett Packard 203 Conference room, 301 Metro Center Blvd., Warwick, RI 02886. In addition to the Public Notice that will be posted in the Providence Journal, the State will also have a second Public Comment Forum onTuesday, May 5, 2015 from 4pm-6pm at the DaVinci Center, 470 Charles Street, Providence, RI ,401-272-7474.

  • HCBS Provider Self Assessment Tool - The State of Rhode Island continues to develop a Statewide Transition Plan in order to become in compliance with the Final Rule for Home and Community Based Services (HCBS).  An integral component of the transition plan is a provider self-assessment/plan of existing settings to determine how closely they currently comply or don’t comply with the Final Rule and if they don’t comply, what is needed in terms of a plan and compliance by March 2019.

    Below are the Provider Self-Assessment tools for Residential and Non-Residential settings that have been developed by the State team.  We are seeking your input and welcome your review and comments.
