To file an appeal regarding Medicaid and Purchased Health Coverage through HealthSource RI, call HealthSource RI at 1-855-840-HSRI (4774). For questions about filing an appeal for human services programs such as SNAP, RIW, Child Care, GPA, or SSP call the Department of Human Services at 1-855-MY-RI-DHS (1-855-697-4347).To request a fair hearing in person, please visit your nearest DHS office. MMP Transition Plan Medicare-Medicaid Plan (MMP) Transition The Integrated Care Initiative (Rhode Island’s Financial Alignment Initiative Demonstration) is a temporary demonstration between the State of Rhode Island, Centers for Medicare and Medicaid Services (CMS) and Neighborhood Health Plan of Rhode Island (NHPRI), designed to improve the care and quality of life for dually eligible Rhode Islanders (people eligible for both Medicaid and Medicare). The program today, administered by NHPRI is referred to as the Neighborhood INTEGRITY Medicare-Medicaid Plan (MMP). On May 9th, 2022, CMS published a Final Rule , that addressed the termination of the Financial Alignment Initiative Demonstration and provided a pathway for States to transition their MMP into an integrated dual eligible special needs plan (D-SNP). Under a provision within the Final Rule, States can maintain their existing MMP through a 2-year extension until December 31st , 2025, so long that the State has provided CMS with a transition plan (“MMP Transition Plan”) by October 1st, 2022. The MMP Transition Plan is a living document that will continue to evolve based on CMS and stakeholder input. The materials posted below provide additional context and information regarding Rhode Island’s transition of the MMP into a fully integrated D-SNP model.
